This can be done at the\u00a0<a href=\"http:\/\/stjohns.floridahealth.gov\/programs-and-services\/clinical-and-nutrition-services\/immunizations\/index.html\"><strong>Department of Health<\/strong><\/a>\u00a0in St. Augustine or private physician. This is state-mandated and must be completed for your student to start school.<\/li><li><strong>Birth Certificate<\/strong><\/li><li><strong>Social Security Card<\/strong>\u2013 this is not required but recommended as is needed for Bright Futures Scholarship.\u00a0 In addition, Colleges, and Universities recommend it be on student transcripts.<\/li><li><strong>Legal Guardianship \/ Custody papers<\/strong>(only if applicable)<\/li><li><strong>Copy of EP, IEP, or\u00a0504 (<\/strong>only if applicable)<\/li><li><strong>Residency Documents \u2013 <\/strong>Click <a href=\"https:\/\/www.stjohns.k12.fl.us\/student\/residency\/\">HERE<\/a> for all required documents<\/li><li><strong>Students school records<\/strong> &#8211; official or unofficial transcripts, report card, transfer grades.
