HEALTH CLINIC INFORMATION AND MEDICAL FORMS

 STUDENT ILLNESS:

We will contact a parent/guardian regarding student pick-up/dismissal if a student has a temperature of 100.4 ° or greater, diarrhea and/or vomiting, significant pain, repeated complaints, or anything out of the ordinary. If students dismiss home with a fever, they must be fever free for 24 hours before they return. This means that they should not return the next day. The 24-hour period begins from the time they no longer have a fever. If you are not sure if you should keep your student out of school due to illness, review the following link: ILLNESS GUIDELINES.

 DISMISSING FROM THE CLINIC:

Student walkers/drivers must have VERBAL PARENT/GUARDIAN PERMISSION to dismiss early due to illness. STUDENTS ARE NOT PERMITTED TO DISMISS WITH ANOTHER STUDENT unless that student is a sibling, and a parent/guardian has given verbal consent.

Dismissing from the clinic does not guarantee an excused absence since often dismissal is at the parent’s discretion and not required by the nurse. Please follow all attendance guidelines for excusing student absences.

The clinic is NOT a backup for early dismissal due to failure to submit an early dismissal request in time. Please plan accordingly.

Medication Policy

Please complete the appropriate medication form(s) (see links below) for your student to take medications at Beachside High School.

PRESCRIPTION MEDICATIONS:

  • If your student requires prescription medication to be kept in the school clinic, this form must be completed and signed by the physician and the parent/guardian: Rx Medication Authorization Form
  • PRESCRIPTION LABEL INSTRUCTIONS MUST MATCH PHYSICIAN INSTRUCTIONS ON MEDICATION AUTHORIZATION FORM IN ORDER TO BE ACCEPTED AT DROP-OFF!
  • ALL prescription medications must remain in the clinic, except for inhalers, EpiPen/Auvi-Q auto-injectors, emergency Glucagon, and emergency seizure medications. Students found in possession of medications without the proper paperwork on file will be subject to disciplinary action as noted in the Student Code of Conduct.

OVER-THE-COUNTER MEDICATIONS:

  • ARE NOT AVAILABLE IN THE CLINIC TO BE DISPENSED TO STUDENTS!
  • Students may self-carry over-the-counter medications by submitting this completed form to the clinic and always keeping a copy with them. Self-Administration of Non-Prescription (OTC) Medication Form
  • All OTC medications must be in the original packaging (NO loose pills).

ELEVATOR PASSES:

Elevator passes are issued through the clinic upon receipt of a written note from a healthcare provider stating the reason for and the length of time the use of the elevator is necessary.

END OF YEAR PROCEDURE

In anticipation of the end of the school year, please plan to pick up your student’s medications/supplies in the clinic by the last day of school 5/30/2025 before 4:00 pm. School policy does not allow for medications to remain in the clinic over the summer. Students may pick up and transport their emergency medications. A parent/guardian must pick up/transport any other type of prescription medication such as ADD/ADHD medication. If you cannot pick up your student’s medication by the end of school day 5/30/2025, please call the clinic to make other arrangements. We will discard any medication left in the clinic after 5/30/2025 according to district policy.

For your convenience, the clinic will be open August 7th and August 8th for medication and paperwork drop-off for the 2025/2026 school year.
